At a temporary worksite, located at Timber Pines Rd 5700 Lot 24 Jacksonville, FL, on the back of the west side of  the 6:12 pitch roof:  On or about December 30, 2025, the employer was exposing an employee to a 12-foot fall hazard, in that the employee was not tied off when installing roofing underlayment material.
