29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a. On the northside exterior of a structure located at 8730 Baymeadows Road at Jacksonville, FL: On or about December 30, 2025, the employer exposed employees to a 16-foot fall hazard, in that employees elevated in a JLG 600S aerial lift installing flashing did not have their lanyard attached to the basket as required.
