29 CFR 1910.22(a)(1):  The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces were kept in a clean, orderly, and sanitary condition.  a.  On or about January 2, 2026, at the above-mentioned workplace, the employer did not ensure that the ladder and upper walking-working platform for the inside Columbus McKinnon (CM) Corporation Primary Hybrid, Serial #PHS17005 rubber tire shredder was kept in a clean and orderly condition.  Employees required to perform preventative maintenance activities were thereby exposed to trip and fall hazards.   b.  On or about January 2, 2026, at the above-mentioned workplace, the employer did not ensure that the walking-working surfaces near the inside shredder's operator control panel and inclined conveyor system were kept in a clean and orderly condition.  Employees were thereby exposed to slip, trip and fall hazards due to excess tire scrap, standing water and ice.

29 CFR 1910.147(d):  The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   a.  On or about January 2, 2026, at the above-mentioned workplace, employees were exposed to amputation hazards from an energized, unguarded ingoing nip point roller during service and maintenance activities, specifically de-icing, of the inclined Columbus McKinnon (CM) Corporation Primary Hybrid, Serial #PHS17005 r rubber tire shredder's conveyor system.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.

29 CFR  1910.219(e)(3)(ii):  All guards for inclined belts were not arranged in such a manner that a minimum clearance of seven (7) feet is maintained between belt and floor at any point outside of guard.  a.  On or about January 2, 2026, at the above-mentioned workplace, the employer failed to ensure guards were installed on the underside of the inclined Columbus McKinnon (CM) Corporation Primary Hybrid, Serial #PHS17005 rubber tire shredder's associated conveyor belt.  Employees operating  equipment and in the vicinity of the unguarded conveyor belt were exposed to caught-in hazards associated with moving parts.  K & S Tire Recycling, Inc. was previously cited for a Serious violation of this Occupational Safety and Health Standard, 29 CFR 1910.219(e)(3)(ii)), which was contained in OSHA inspection number 1812074, citation number 2, item number 5, and was affirmed as final order on June 6, 2025, with respect to a workplace located at 303 State Street, Chicago Heights, IL 60411.

29 CFR 1910.303(g)(1):  Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a.   On or about January 2, 2026, at the above-mentioned workplace, the employer did not ensure that the working space around electrical panel distribution cabinets and boxes were provided and maintained at least three feet of clearance.  Employees were thereby exposed to trip and fall hazards associated with blocked and/or obstructed electrical panel boxes.  K & S Tire Recycling, Inc. was previously cited for a Serious violation of this Occupational Safety and Health Standard, 29 CFR 1910.303(g)(1), which was contained in OSHA inspection number 1812074, citation number 2, item number 7b, and was affirmed as final order on June 6, 2025, with respect to a workplace located at 303 State Street, Chicago Heights, IL 60411.
