On or about December 16, 2025, at a job-site located at 1920 Sherwood Ave NW, Massillon Ohio 44646; the employer did not ensure employees were adequately protected from rolling and or falling debris from a spoil pile that was located directly at the edge of the West side of the trench. An employee was working in the 7 foot deep trench servicing/installing sanitary line directly below the loose material at the edge of the trench.

On or about December 16, 2025, at a job-site located at 1920 Sherwood Ave NW, Massillon Ohio 44646; while working in a trench the employees utilized a portable extension ladder that was broken in half to gain access/egress, the employer did not ensure that employees utilized the extension ladder properly.
