29 CFR  1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) On or about January 5, 2026; 2901 Minnesota Ave SE, westside, the employer did not ensure that employees were wearing the required eye protection while operating portable power tools. Employees were observed using portable drills exposing them to potential flying debris during the process.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about January 5, 2026; 2901 Minnesota Ave SE, westside; employees installing a sign while standing on the roof of a company van were exposed to a fall hazard of approximately 7 feet to the ground below. Employees were not protected by a personal fall arrest system during the task.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. Employees were observed working at an estimated height of 7 feet above ground level with fall protection. The step ladder was not open and locked in accordance with the manufacture requirements. The improper use of the equipment exposed the employee to an estimated 8-foot fall to ground level. The lack of training exposed the employees to an estimated fall of 7 feet.
