29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): On or about December 17, 2025, and thereafter, at the work site located at 401 West Fields BLVD, Eldorado Spring, Mo 64744: Employees were exposed to caught in hazards while conducting machine operating and maintenance activities. Employees were using machines with unguarded pulleys with parts to make HVAC parts and pieces. The employer did not ensure that Pulleys with parts seven feet or less from the floor or work platform were not guarded. Instances of exposure include but are not limited to: Instance 1: Unguarded pulleys with parts located on the 500 line. Instance 2: Unguarded pulleys with parts located on the Collar machine.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. At the work site located at 401 West Fields BLVD, Eldorado Spring, Mo 64744, the employer did not report an amputation as a result of a work-related incident to OSHA within twenty four (24) hours. An employee amputation occurred on or about December 17, 2025. The amputation was reported via complaint on December 29, 2025.
