29 CFR  1910.147(d)(2): An orderly shutdown of machine or equipment was not utilized to avoid any additional or increased hazards to employees as a result of equipment stoppage:  At 30 Arrowhead Industrial Blvd, St Peters, MO, 63376, On December 23, 2025, and at times prior thereto, employees were exposed to hazards associated with the sudden reenergization of equipment due to not performing an orderly shutdown of a dust collection fan assembly at dust collector 1, before servicing a sensor.   An employee had four fingers partially amputated when cleaning a sensor in close proximity of a fan assembly which restarted when the sensor was cleaned.
