On or about December 31, 2025, a forklift operator was changing a propane fuel cylinder on a power industrial truck without hand protection. The worker was exposed to potential skin burns (frostbite) that could have resulted from contact with liquid propane.

An operator stopped his forklift in the middle of a passageway to change out a propane fuel cylinder when he was struck by a second forklift, crushing his leg between the two forklifts.

On or about December 31, 2026, a forklift operator was struck by another forklift at the facility located at 205 Soccer Park Road in Fenton, MO. An operator stopped his forklift in the middle of a passageway to change out a propane fuel cylinder. A second forklift operator was moving in reverse and not looking the direction of travel struck the first operator crushing his leg between the two forklifts.
