29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On or about December 31, 2025, the employer failed to provide frequent and regular job site inspections, exposing employees to hazards associated with jacking foundations and installation of Helical piers. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.21(b)(2):The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about December 31, 2025, the employer failed to train field employees on the workplace hazards or the regulations required to control or eliminate the hazards, thereby exposing employees to caught in and struck-by hazards associated with foundation jacking and operating a with a Helical Pier machine. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
