1910.27(a) Scaffolds. Scaffolds used in general industry must meet the requirements in 29 CFR part 1926, subpart L (Scaffolds): 29 CFR 1926.451(d)(3):The inboard ends of suspension scaffold outrigger beams shall be stabilized by bolts or other direct connections to the floor or roof deck, or they shall have their inboard ends stabilized by counterweights, except masons' multi-point adjustable suspension scaffold outrigger beams shall not be stabilized by counterweights. On or about December 17, 2025, during a window cleaning project located at 2630 13th Street, Cuyahoga Falls, Ohio 44223, the employer permitted the use of a portable rooftop roller (outrigger beam) to support a boatswain's chair scaffold that was not stabilized or secured in place against movement. The outrigger beam was not connected to the roof deck by bolts or other direct connection, nor was it tied back, anchored, braced, or otherwise secured to prevent displacement while supporting an employee descending into the courtyard area of the building from a height greater than 21 feet, exposing the employee to a fall hazard.

1910.27(a) Scaffolds. Scaffolds used in general industry must meet the requirements in 29 CFR part 1926, subpart L (Scaffolds): 29 CFR 1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system; On or about December 17, 2025, during a window cleaning project located at 2630 13th Street, Cuyahoga Falls, Ohio 44223, the employer did not ensure that the onsite supervisor working from a boatswain's chair scaffold was protected by a personal fall arrest system. Although the supervisor was wearing a body harness, the harness was not connected to an anchorage while descending into the courtyard area of the building greater than 21 feet above the courtyard ground level. When the rooftop roller support system failed, the supervisor fell to the ground below, resulting in multiple fractures.
