29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) 331 Maple Ave, Hanover, PA 17331; in the maintenance building - On or about January 23, 2026, the tongue guard opening on the bench grinder at the maintenance bench was measured at 3/4 inches instead of the 1/4 inch as required. Employees were exposed to a struck by hazard. No abatement certification required.
