29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  At the workplace located at 3030 South 24th Street, Suite B, Kansas City, Kansas, employees engaged in automotive interior maintenance and upholstery activities, handling contact adhesive, silicone sealant and vinyl/plastic cleaner, were exposed to chemical hazards. The employer did not provide employees with effective information and training on the chemical hazards associated with the handling of DAP Weldwood, GE SCS1000 and Crest Vinyl/Plastic Kleener. The chemical hazards associated with these chemicals include: skin, eye and respiratory tract irritation, drowsiness and/or dizziness, may cause damage to organs through prolonged or repeated exposure and is harmful or fatal if swallowed.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  At the workplace located at 3030 South 24th Street, Suite B, Kansas City, Kansas, employees engaged in automotive interior maintenance at the facility were exposed to respiratory hazards. Employees were permitted to voluntary use N95 respirators in the workplace and were not provided the information contained in Appendix D. An N95 respirator was observed improperly stored on a tool rack at the roofer workstation.
