29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:  At the workplace located at 2338 Merriam Ln. Kansas City, Kansas, employees engaged in sign manufacturing activities, handling isopropyl alcohol and adhesive remover, were exposed to chemical hazards. The employer did not ensure that each employee utilize necessary personal protective equipment such as protective gloves and safety glasses to protect employees from chemical hazards. The safety data sheets (SDS) for Ideal Isopropyl Alcohol and Rapid Remover indicate these chemicals are skin and eye irritants that require the usage of hand and eye protection.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  At the workplace located at 2338 Merriam Ln. Kansas City, Kansas, employees engaged in material handling activities, operating the forklift, were exposed to struck-by hazards. The employer did not ensure that each operator had successfully completed training consisting of a combination of formal instruction, practical learning and evaluation required by the standard, prior to operating the Komatsu FG25ST-14 forklift.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  At the workplace located at 2338 Merriam Ln. Kansas City, Kansas, employees engaged in sign manufacturing and installation activities, handling isopropyl alcohol, adhesive remover, metal working lubricant and structural adhesive, were exposed to chemical hazards. The employer did not provide employees with effective information and training on the chemical hazards associated with the handling of Ideal Isopropyl Alcohol, Rapid Remover, UNIST Coolube 2210 and H.B. Fuller Vibra-TITE MP-55420. The chemical hazards associated with these chemicals include: skin, eye and respiratory irritation, drowsiness and/or dizziness, may cause an allergic skin reaction and may be fatal if swallowed and enters airways.
