29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: At a new multi-family residential worksite located at 8800 N Ames Dr, KC MO -- an employee working from an aerial lift was wearing his safety harness with lanyard. However, the employee was not connected to the aerial lift. This resulted in exposing the employee to being catapulted from the aerial lift causing serious injury and/or death. This was documented on or about 1/6/2026.
