29 CFR 1926.416(a)(1): The employer permitted an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work and was not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means: a) On or about December 15, 2025, employees performing demolition work to an existing commercial structure were permitted to work in proximity to energized circuit breaker boxes. b) On or about December 15, 2025, employees performing demolition work to an existing commercial structure were permitted to work on live electrical wiring throughout the building. No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels by personal fall arrest systems, covers or guardrail systems erected around such holes: a) On or about April 1, 2026, employees were performing demolition work and exposed to floor holes on the first, second, third and fourth floors which created a fall hazard to the basement. No abatement certification or documentation is required for this item.
