29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On 1/8/2026 at worksite located at 22717 Nebula Way #2604, Land O Lakes, FL 34637: Employee's were exposed to fall hazards of approximately 10 ft 2.5 inches in that they were performing framing operations on the second floor of a building with no fall protection.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) On 1/8/2026 at worksite located at 22717 Nebula Way #2604, Land O Lakes, FL 34637: Employee's were exposed to fall hazards of approximately 10 ft 2.5 inches in that they were working on the second floor and the fiberglass A-Frame ladder that they were using to access/egress the second floor was approximately 4 ft 5.5 inches short of the landing surface.

29 CFR  1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  a) On 1/8/2026 at worksite located at 22717 Nebula Way #2604, Land O Lakes, FL 34637: Employee's were exposed to fall hazards of approximately 10 ft 2.5 inches in that they were using a ladder to access and egress the second floor of a new construction residential building that was approximately 4 ft 5.5 inches short of the landing surface without training.
