29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. (a) Worksite - Roof: On or about 1/8/2025, employees were exposed to fall hazards while engaged in roofing activities from a height of approximately 40' where one anchorage point was being used by three employees to attach three rope lanyards. (b) Worksite - Roof: On or about 1/8/2025, employees were exposed to fall hazards while engaged in roofing activities from a height of approximately 40' where one anchorage point was being used by three employees to attach three rope lanyards. The roof anchor was attached to the roof with only 6 screws instead of 12.

(a) Worksite - Roof: On or about 1/8/2025, an employee was exposed to fall hazards while performing roofing repair activities at a height of approximately 40 feet, with the employee's rope grab not positioned to limit the fall to 6 feet or less.
