1910.147(c)(7)(i)(A) Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolate: (a) On or about September 24, 2025, employees working on and around the Advanced Batch Mixer (M:ABM 1000; S/N 0850811) were exposed to an amputation hazard, the employer did not ensure the exit chute was adequately guarded to prevent the operator from reaching into the point of operation.
