29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about January 8, 2026, at the above addressed jobsite, an employee was conducting sewer main connection operations in a trench measured at approximately seven (7) feet six (6) inches in depth. The trench was not provided with cave in protection, thereby exposing the employee to cave in hazards.
