Location: a) On jobsite: Exterior wall of western side of the building:   On or about January 9, 2026, employees erected a pump?jack scaffolding system, which was used to install the siding on the western wall of the building. The scaffold was not erected by experienced and trained employees under the supervision and direction of a competent person.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  Location: a) On jobsite: Exterior wall of western side of the building:  On or about January 9, 2026, two employees were working on a pump?jack scaffold platform. The employees were not protected from falling to lower levels through the use of an appropriate fall protection system. The employees were exposed to a fall hazard of approximately 18 feet to the ground level.

Location: a) On jobsite: Exterior wall of western side of the building:  On or about January 9, 2026, the employer did not ensure that a guardrail system was installed along all open sides and ends of the pump-jack scaffold platform used by employees performing siding installation. The employees were exposed to a fall hazard of approximately 18 feet to the ground level.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Location: a) On jobsite: Exterior wall of western side of the building:   On or about January 9, 2026, the employer did not provide training to employees who erected and worked from a pump-jack scaffold built with incomplete railings. Employees could not recognize hazards associated with the scaffold and were exposed to a fall hazard of approximately 18 feet from the scaffold platform to the ground below.

No guardrail system, safety net system, or personal fall arrest system was provided or in use. The employees were exposed to a fall hazard of approximately 25 feet from the roof to the ground below.

Location: a) On jobsite: Exterior wall of western side of the building  On or about January 9, 2026, employees used a extension ladder to access their work area. The ladder used did not extend 3 feet above the intended landing surface.
