29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) Jobsite: 118 Frederick Street, Hanover, PA. On or about January 9, 2026, employees were working in a trench approximately 13 feet deep and were not provided with a safe means of egress. The ladder present was not positioned to provide a usable means of egress and did not extend to the top of the trench, exposing the employees to a caught-in or struck-by hazard.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1926.651(l):Guardrails complying with 1926.502(b) were not provided where walkways were 6 feet (1.8m) or more above lower levels:  a) Jobsite: 118 Frederick Street, Hanover, PA. On or about January 9, 2026, an employee crossed over an approximately 13-foot-deep excavated basement by walking on a wood plank used as a bridge from the blacktop surface to the top of a concrete form wall. The walkway was not provided with guardrails, exposing the employee to a fall hazard of approximately 13 feet to the lower level.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  a) Jobsite: 118 Frederick Street, Hanover, PA. On or about January 9, 2026,  employees installing foundation wall concrete forms were working inside a straight cut trench approximately 13 feet deep without cave-in protection, exposing employees to an engulfment hazard.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

On or about January 9, 2026, employees installing concrete foundation walls were exposed to impalement hazards from nine unguarded vertical reinforcing steel bars.  b) Jobsite: 118 Frederick Street, Hanover, PA. On or about January 9, 2026, employees working on walking/working surface walls were exposed to impalement hazards from nine unguarded vertical reinforcing steel bars.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
