29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Jobsite at or near 20 West Ridley Ave, Ridley Park, PA - On or about January 9, 2026, employees  were conducting roofing activities 6 feet or more above lower levels without fall protection exposing them to fall hazards.

29 CFR  1926.503(a)(1):Employer did not provide fall protection training to those employees who might be exposed to a fall hazard:  a)Jobsite at or near 20 West Ridley Ave. Ridley Park,  PA - On or about January 9, 2026, the employer did not provide fall protection training to employees who were exposed to fall hazards.  Employees performed roofing removal and replacement activities without being trained on fall protection, thereby exposing employees to fall hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  a) Jobsite located at 20 West Ridley Ave. Ridley Park, PA. - The employer did not ensure the side rails of a portable ladder used for access to an upper landing surface extended at least 3 feet above the upper landing surface being accessed. Employees utilized a portable ladder to access a second and third story roof without having side rails extended at least 3 feet above the landing surface, thereby exposing employees to fall hazards.  Condition was noted on or about January 9, 2026
