29 CFR  1910.22(a)(2): The employer did not ensure that the  floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.   a) Production Area: On 11/18/2025, and at times prior, the employer exposed employees to slip and fall hazards in that, the employer failed to ensure that the floor of working area were kept in a clean and dry condition when the water was not removed on the floor around the machines.

29 CFR  1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator had been involved in an accident or near miss incident:   a) Production Area - On or about  01/12/2026, and at times prior, the employer exposed employees to struck by hazards in that, the employer failed to ensure that an employee was provided with the refresher training after being involved in a forklift incident when employee was required to continue to operate the forklift.
