29 CFR 1910.178(n)(11): Dockboard(s) or bridgeplate(s) were not properly secured before they were driven over: a) On or about 02/23/2026, on the Loading Dock, when an employee operating a powered industrial vehicle (Raymond) entered an MVS trailer, the dock plate was not secured and collapsed jolting the Raymond forward and causing the wheels to fall into the dock plate area. Employee was exposed to injury from incident. NO ABATEMENT CERTIFICATION REQUIRED
