29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, or fittings. Conductors entering boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, and fittings shall also be effectively closed. On or about January 12, 2026, and again on January 13, 2026, at the temporary worksite located at 4671 Michaels Drive, Appleton, Wisconsin, workers were exposed to potential contact with electrical current due to fittings not effectively closed on a Square D Load Center panel box used for temporary power. Specifically: Instance a: The openings of unused circuit breaker slots were covered by cardboard and duct tape and were not effectively closed. Instance b: A knockout located on the top of the circuit breaker box was open and not effectively closed.
