29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level. a) Pilot Fuel Station, 30 Benvenue Rd, Duncannon, PA: On or about January 13, 2026, two employees were working from a scaffold that measured in excess of 10 Feet 6 Inches to the underside of the work platform without any fall protection system in use. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.452(k)(5):Scaffold platforms were bridged on to another. a) Pilot Gas Station, 30 Benvenue Rd, Duncannon, PA: On or about January 13, 2026, two employees were exposed to a collapse hazard while working from two (2) platforms of a ladder jack scaffold that were bridged together at the center. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
