29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Outside Building #9, 60 White Deer Run Rd, Allenwood, PA 17810 - On December 28, 2025, an employee (security guard) was walking to the restroom, when the employee slipped and fell on ice, resulting in a hospitalization. The work-related incident was not reported to OSHA by the company within twenty-four (24) hours. The White Deer Run of Allenwood was previously cited for a violation of this occupational safety and health standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1576760, citation number 1, item number 2 and was affirmed as a final order on March 21, 2022, with respect to a workplace located at 360 White Deer Run Road, Allenwood, PA 17810.
