29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) North end of building; On or about January 13, 2026, hardhats (protective helmets) were not worn by employees who were exposed to overhead and head impact hazards during truss setting and bracing operations.

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: Note: Preamble to Subpart L promulgated new paragraph (c)(2)(iv) & (v) in final rule to provide guidance. Added provision requires that in the case of fork-lifts, the entire scaffold platform be secured to the forklift. (a) North end of building; On or about January 13, 2026, the employer did not ensure that an elevated work platform manufactured by Star Industries was properly secured to the Caterpillar rough-terrain forklift (model TL642D) with fork retention pins and carriage safety restraint chains to prevent the platform from separating from the forklift during use.
