29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) 5101 Carlisle Pike, Mechanicsburg, PA - On or about January 13, 2026, 5 employees were exposed to a fall of approximately 10 feet to pavement when transiting between the 16-foot Werner aluminum extension ladder and the roof of the 1-story commercial building due to the ladder side rails did not extend 3-feet above the roof edge.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   (a) 5101 Carlisle Pike, Mechanicsburg, PA - On or about January 13, 2026, 5 employees were not protected from a fall of approximately 10 feet to pavement by means of a fall protection system while removing and replacing roofing material to repair a water leak.  Golden Hammer Roofing, LLC was previously cited for a violation of a substantially similar occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1779622, citation number 1, item number 1 and was affirmed as a final order on 12-17-2024, with respect to a workplace located at 1220 Sheridan Street, Williamsport, PA 17701.  No abatement certification and documentation required.

29 CFR  1926.503(a)(2):The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  (a) 5101 Carlisle Pike, Mechanicsburg, PA - On or about January 13, 2026, the employer did not ensure that the fall protection training was provided to two (2) employees who were working without a means of fall protection or prevention on the roof which was approximately 10 feet above the pavement.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
