29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a): Exterior Scaffold - East Side - Three employees were exposed to fall hazards of approximately 16 feet when working on scaffolds without proper fall protection. The employees were performing masonry work from the scaffold without proper fall protection, on or about January 14, 2026.  There is no abatement certification or documentation required for this item.

29 CFR  1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee   (a): Exterior Scaffold - East Side - Two employees were exposed to fall hazards of approximately 16 feet when working on scaffolds. The employees were performing masonry work from the scaffolds using personal fall arrest systems with improper anchor points, on or about January 14, 2026.  There is no abatement certification or documentation required for this item.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   (a): Second Floor of Structure - Employees were exposed to fall hazards of approximately 12 feet when working on the second floor. Employees were carrying materials near unprotected sides of the building; guardrail system was not continuous exposing employees to fall hazards, on or about January 14, 2026.  There is no abatement certification or documentation required for this item.
