29 CFR 1910.25(b)(1):The employer did not ensure that handrails, stair rail systems, and guardrail systems were provided in accordance with 29 CFR 1910.28: Hearst Hacienda Lodge, ground level storage area: On or about January 12, 2026, Employees were permitted to walk up and down the stair system that did not have a handrail, stair rail systems, and guardrail systems installed on the open side of the stairs to access the maintenance shop located on the second level. Employees were exposed to a 6-foot 10 inch fall.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: Hearst Hacienda Lodge, inside the Safe room: On or about January 12, 2026, a service panel (240 volt) had two unused circuit breaker openings that were not effectively closed, the energized bus bars were exposed. Employees were potentially exposed to electrical hazards.
