29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. Residential apartment project, 427 Farmington Avenue, Plainville, CT: On or about January 16, 2026, employees were exposed to a fall hazard of approximately 21 feet from the height of the aerial lift basket to the ground. The employees were operating in the platform of an Aichi SP460CJ off?terrain telescopic boom lift while performing framing and panel?board sheathing activities without the use of fall protection. Employees did not wear personal fall arrest systems, nor were lanyards attached to the boom or basket of the lift, exposing them to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Residential apartment project, 427 Farmington Avenue, Plainville, CT: On or about January 16, 2026, employees performing carpentry and framing activities on the roof of the structure were exposed to a fall hazard of approximately 21 feet from the unprotected roof edge to the ground. No guardrails, safety nets, or personal fall arrest systems were provided. Employees were not wearing harnesses and no lanyards were attached to any anchorage, exposing them to a significant fall hazard.
