29 CFR  1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  (c) Press Production Area -  On or about January 14, 2026, the established procedures utilized at the workplace to control the hazardous energy of the Komatsu mechanical press, Model No. OBS60, Serial No. 15621 in the press area did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of the potentially hazardous energy when employees performed servicing or maintenance on machines. The procedure for the Komatsu mechanical press, Model No. OBS60, Serial No. 15621 did not identify potential energy as an energy source for the equipment, and did not require the use of blocks to isolate prevent the unexpected motion in the machine.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed\:  (a)  4-Slide Production Area -  On and before January 14, 2026, the employer did not ensure that energy control procedure for the Torrington Manufacturing Company, Verti-Slide labeled Machine 22, with no accessible serial number on the machine  were evaluated annually to ensure that the procedures and requirements of this standard were being followed. Employees were exposed to amputation hazards during servicing and maintenance procedures including machine setup. (b) 4-Slide Production Area -  On and before January 14, 2026, the employer did not ensure that energy control procedure for the A.H. Nilson Machine Co. wire forming press numbered 4 on the flywheel cover of the machine, with no accessible serial number were evaluated annually to ensure that the procedures and requirements of this standard were being followed. Employees were exposed to amputation hazards during servicing and maintenance procedures including machine setup. 29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a) 4-Slide Production Area -  On or about October 21, 2025, an employee was exposed to machine hazards associated with moving parts during setup of the Torrington Manufacturing Company, Verti-Slide labeled Machine 22, with no accessible serial number on the machine.   The employer did not implement equipment or procedures to isolate the Torrington Manufacturing Company, Verti-Slide labeled Machine 22, with no accessible serial number on the machine from energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving lock-out/tag-out device application [1910.147(d)(4)], dissipation or residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.
