29 CFR  1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a) On and before 1/16/2026, the employer did not establish an energy control program to address the following topics; energy control procedures, employee training and periodic inspections, for equipment such as but not limited to; SIR Machines, Roboformer, B3 Press Brakes and the paint line. Employees were not provided with energy control procedures to utilize, nor were they trained as authorized employees to ensure that they understood the purpose for the machine specific procedures and how to isolate each hazardous energy sources such as but not limited to; electric, pneumatics, hydraulics, & gravity.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or before January 16 2026, the employer did not ensure that the point of operation on machines, such as but not limed to the B3 Press Brake (Ser. # MB_0626) & B3 Press Brake (Ser. # MB_0887), was guarded, in that employees were exposed to caught-in hazards due to the light curtains being disabled and the foot pedals were positioned in such a manner that would allow employees the ability to activate the press while their hands or body parts could be in the point of operation.

29 CFR  1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.   a) On or before January 16, 2026, the employer did not ensure that adequate access and working space were provided and maintained around electrical cabinets and panels, including those located behind the P4 line two and across from the paint oven line (PC3 and PC2). Wooden crates, pallets, and a metal frame had been placed in front of and around the panels, reducing the clearance to less than the required 30 inches.

29 CFR  1910.303(g)(2)(i):Except as�elsewhere required�or permitted�by this�standard, live�parts of�electric equipment�operating at�50 volts�or more�shall be�guarded against�accidental contact by�use of�approved cabinets�or other�forms of�approved enclosures�or by�any of�the following�means:  A) By location in a room, vault, or similar enclosure that is accessible only to qualified persons; (B) By suitable permanent, substantial partitions or screens so arranged so that only qualified persons will have access to the space within reach of the live parts. Any openings in such partitions or screens shall be so sized and located that persons are not likely to come into accidental contact with the live parts or to bring conducting objects into contact with them; (C) By placement on a suitable balcony, gallery, or platform so elevated and otherwise located as to prevent access by unqualified persons; or (D) By elevation of 2.44 m (8.0 ft) or more above the floor or other working surface.  a) On or before January 16, 2026, the employer did not ensure that the 600?volt live electrical components associated with equipment such as the SIR 1 (Ser. # C3161) were guarded to prevent employees from making contact with the energized parts on the transformer.  b) ) On or before January 16, 2026, the employer did not ensure that the 480?volt live electrical components associated with the Main Power Coat (Main Feeder Switchboard #9) were guarded to prevent employees from making contact with energized parts on the electrical cabinet.
