29 CFR 1926.34(a): "General." In every building or structure exits shall be so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it is occupied. a) On or about January 14, 2026, the employer failed to ensure the exit near the security turnstile was clear and free from hazards. A flexible extension cord was crossing the exit route to power the server trailer, exposing employees to tripping hazards in the event of an emergency.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a) On or about January 14, 2026 the employer failed to ensure a flexible extension cable powering the server trailer was protected from accidental damage, thereby exposing employees to shock hazards. The cable ran through the pinch point of the door and was susceptible to damage.
