29 CFR  1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  a) On or about January 13, 2026, at the above addressed jobsite, an employee conducting welding operations from a JLG 800AJ, Serial No. 0300167980, articulating boom lift climbed onto the mid and top rails of the basket to access the roof approximately fifty (50) feet in elevation, thereby exposing the employee to fall hazards.

29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) On or about January 13, 2026, at the above addressed jobsite, employees conducting welding operations from a JLG 800AJ, Serial No. 0300167980, articulating boom lift were not connected to the boom or basket, thereby exposing the employees to the hazards associated with falls of approximately fifty (50) feet.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) On or about January 13, 2026, at the above addressed jobsite, an employee conducting welding operations was exposed to falls of approximately fifty (50) feet in elevation, created by the presence of unprotected sides.  b) On or about January 13, 2026, at the above addressed jobsite, an employee working below welding operations was exposed to falls of approximately twenty-five (25) feet in elevation, created by the presence of unprotected sides.
