Slip-resistant feet shall not be used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery:  On or about January 15, 2026, at a worksite located at 924 N Park View Circle in Mosinee, Wisconsin, an extension ladder was used on a polished concrete surface which was capable of becoming slippery. The Keller 24-foot ladder's (Model 5124) slip-resistant feet were used as a substitute for care in placing, lashing, or holding the ladder on the polished concrete floor as employees utilized the ladder to access the upper level approximately 10.8 feet above the ground.

(a) On or about January 15, 2026, a Keller 24-foot ladder (Model 5124) was used to access an upper-level work area approximately 10.8 feet above the ground and immediately within the breakroom door's swing path.  (b) On January 21, 2026, a Keller 28-foot ladder (Model 5128) was used to access an upper-level work area approximately 10.8 feet above the ground and immediately within the breakroom door's swing path.
