On or about January 20, 2026, at the site: employees were exposed to 14-foot hazards while conducting framing activities from a man made scaffold box platform, in that fall protection was not used and the box platform was not secured to the class 7 rough terrain Manitou MTA 12055 Series 3 telehandler forks or the load back rest.
