29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: Location: 300 New State Hwy, Raynham, MA 02767 On or about January 18, 2026, employees were exposed to crushed-by hazards where a pallet of forty-nine 50 lb. bags of rock salt was stacked on top of a pallet of cases of 16 oz. bottled water and not secured against sliding or collapse when loaded at the Shaw's Wells Distribution Center. Later a third party delivery employee was crushed by the unstable materials at the delivery location.

29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: Location: 300 New State Highway Raynham MA. 02767 On January 18 2026, employees were exposed to crushed-by hazards where a store employee operated a powered pallet jack to pull a palletized load of bottled water and rock salt from a trailer without first confirming that the load could be safely moved, and without verifying that no workers were in the zone of danger. As a result, an employee suffered serious injuries from being crushed when the load was moved and the unstable pallet collapsed on him.
