29 CFR 1910.147(d)(5)(i): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. a) On or about January 5, 2026, at Thermoformer #4 in the production area, the employer failed to ensure that all gravity-stored energy in the bottom platen was fully relieved or restrained before employees performed service and maintenance activities. The condition exposed employees to crushing and caught-between hazards from uncontrolled gravity-stored energy. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
