(b)Royal Plastics, Inc. at 200 32nd Avenue, Brookings, SD 57006:  On or about and at times prior to January 21, 2026, the employer did not ensure adequate guarding was in place on the Roll-In All-Purpose Vertical Bandsaw, Model Number: SF8590, Serial Number: 80241 in that five inches of blade was exposed on the aft column of the equipment.  This condition exposed employees to amputation hazards.
