29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a)Royal Plastics, Inc. at 200 32nd Avenue, Brookings, SD 57006: On or about and at times prior to January 21, 2026, the employer did not ensure that the unused portion of blade on the Roll-In All-Purpose Vertical Bandsaw, Model Number: SF8590, Serial Number: 80241 was properly adjusted, in that more than four inches of saw blade was exposed while cutting rectangular bar stock. This condition exposed employees to amputation hazards. (b)Royal Plastics, Inc. at 200 32nd Avenue, Brookings, SD 57006: On or about and at times prior to January 21, 2026, the employer did not ensure adequate guarding was in place on the Roll-In All-Purpose Vertical Bandsaw, Model Number: SF8590, Serial Number: 80241 in that five inches of blade was exposed on the aft column of the equipment. This condition exposed employees to amputation hazards.
