29 CFR  1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were dispensed into containers without the nozzle and container being electrically interconnected.  a)  Mack Trucks - Lehigh Valley, PA -  On or about January 20, 2026, employees were dispensing Purge Solvent, a Category 2 flammable liquid, into a solvent tank which then discharged into an open bucket in the paint booth, exposing employees to fire and explosion hazards.  The containers were not properly bonded and grounded as required by this paragraph.  No abatement certification or documentation required.

OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that spray areas were not kept as free from the accumulation of deposits of combustible residues as practical, with cleaning conducted daily if necessary.   a)  Mack Trucks - Lehigh Valley, PA -  On or about January 20, 2026, the employer was utilizing a ventilation booth to control spray paint vapors.  The employer did not keep floors and walls of the spray booth free from the accumulation of combustible overspray residue, exposing employees to fire and explosion hazards.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR 1910.134(k): The employer did not provide comprehensive, understandable, and effective annual training to employees who were required to use elastomeric respirators.   a)  Mack Trucks - Lehigh Valley, PA -  On or about January 20, 2026, employees painting chassis were required to wear an airline supplied, full-face, hood respirators during spray painting in the spray booth.  The employer did not provide effective annual training on respirators in accordance with this section.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
