29 CFR  1910.178(a)(4):  Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   a) ATCO RUBBER PRODUCTS, INC.- Raw material storage Area/production area:  Employees were operating the Yale propane forklift Model #GLC050VXNEAE083, S/N C910V13854U, Cat forklift Model #2C4000, S/N AT9043457, Cat forklift Model # 2C4000, S/N AT9047253, Cat forklift  Model 2C4000 S/N A19048099, with forklift attachments to clamp/move big rolls of raw materials.  The attachments did not have any markings and were used without obtaining the forklift manufacturer's written approval for the attachment usage, occurred on or about 01/21/2026.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) ATCO RUBBER PRODUCTS, INC.-  Line 1/Production area:  The top cage on the Ram Packer machine (20 inches - Line 1) was not properly guarded, creating a hazard where employees could hit their heads when the cage lowers while they are extending product to prevent jams, observed on or about 01/21/2026.  b) ATCO RUBBER PRODUCTS, INC - Manufacturing lines 1 to 9:  Employees unloading the rolls of insulation and then threading them through the table/conveyor are exposed to unguarded rolls that help push the flat insulation sheets through the shear, exposing them to pinch points., occurred on or about 01/21/2026.
