29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.  At the worksite located at 2065 E Pythian Street, Springfield, MO an employee working off a mobile scaffold climbed the guard railing to gain access to a upper work surface, exposing them to falls greater than 14 feet when the employer did not ensure each employee on a scaffold more than 10 feet above a lower level was protected from falling to that lower level . This was documented on or about 1-21-2026.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At the worksite located at 2065 E Pythian Street, Springfield, MO, an employee working on top of a compressor room was exposed to fall hazards greater than 14 feet to a lower level when the employer did not ensure each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. 29 CFR  1926.1053(b)(6):Ladders were used on surfaces which were not stable and level:  At the worksite located at 2065 E Pythian Street, Springfield, MO an employee working on top of a compressor room was exposed to fall hazards from a portable ladder that was placed on a unstable piece of plywood and not secured against accidental displacement, when the employer did not ensure ladders were used on surfaces that were stable and level. This was documented on 1-21-2026.
