29 CFR 1926.102(a)(1): The employer had not ensured each affected employee had used appropriate eye and face protection when exposed to eye or face hazards from gases or vapors or potentially injurious light radiation:  On or about January 21, 2026, AA Installation LLC performing roofing work at 8506 Titchfield Court in Affton, Missouri, did not ensure each employee exposed to eye hazards from flying particles or debris, while operating  pneumatic nail guns, utilized eye protection.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  On or about January 21, 2026, at a worksite located at 8506 Titchfield Court in Affton, Missouri, employees of AA Installation LLC  were exposed to fall hazards while working at heights of 18 feet above the lower level without the use of a fall protective system.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:   On or about January 21, 2026, AA Installation LLC, was performing roofing work at 8506 Titchfield Court in Affton, Missouri. The employer did not ensure a Werner extension ladder used by employees for accessing and egressing the roofing work was properly positioned so the siderails extended three feet past the upper landing surface.

29 CFR  1926.1053(b)(3): Ladder was loaded beyond the maximum intended load for which they were built or beyond the manufacturer's rated capacity.  On or about January 21, 2026, employees of AA Installation LLC were installing new roofing shingles on a three story apartment building located at 8506 Titchfield Court in Affton, Missouri. The employer exceeded the manufacturers rated capacity of a Werner aluminum extension ladder by allowing two employees to descend the ladder at the same time which exposed employees to fall hazards.

29 CFR  1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive side rails:  On or about January 21, 2026,  AA Installation LLC, was performing roofing work at 8506 Titchfield Court in Affton, Missouri. The employer at the site utilized a Werner aluminum extension ladder in a manner that could have caused a employee or conductive side rails to contact a 240 volt electrical drop line exposing employees to shock and burns.
