OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by and roll over hazards:  a) On or about January 22, 2026, at jobsite 15575 Tamiami Trail, Naples, FL 34114, employees were exposed to struck-by and roll over hazards during the operation of a Komatsu wheel loader (Model number: WA320-8; SN: A38918) with a damaged front windshield, damaged front windshield wiper blade, and non-operational lights.
