29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment  was  isolated from the energy source and rendered inoperative:  a):Production area:   Employees were exposed to hazardous energy while perfoming maintenance on  equipment , including, but not limited to changing the  blade on  the   Reliant Model # DD90, Serial # 1174802 Band saw machine .  On or about 01/22/26.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1910.178(l)(6): The employer did not certify that each operator was  trained and evaluated as required by this paragraph (l) and did not document the training through a  certification that included the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.   a):  Storage area: An employee operating  the  CAT GC15K serial # AT81C03269 forklift , was not evaluated and certified prior to forklift use.  On or about 1/22/26.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
