(a) On or about January 21, 2026, an opening in the service envelope of the KPI 4250 that measured approximately 42in x 50in was not effectively covered to prevent employees working in the area from tripping into or stepping into the uncovered portions of the opening.

(a) On or about January 21, 2026, a Milwaukee M18 FUEL 4-1/2" / 5" Grinder, was being used in an unsafe condition and against the manufacturers' recommendations by using a 6 inch CGW Abrasives cutoff wheel which exposed employees to caught in and struck by hazards.

29 CFR  1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel.  (a) On or about January 21, 2026, an employee was operating a Milwaukee M18 FUEL 4-1/2" / 5" Grinder with the safety guard removed to cut bolts, thereby exposing the employees to caught in and struck-by hazards.
