A) 116 South Avenue Middlesex, NJ 08846  Employees were being allowed to stand on the forks of a Hyster H50A forklift and be lifted into the back of trailers that were being loaded with pallets. This condition exposed employees to fall hazards.  Hazard noted  on or about 01/22/26
