29 CFR 1926.102(a)(1):Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about January 21, 2026 at a work location located at 5095 Brainard RD, Solon Ohio 44139, the employer did not ensure that employees used eye protection to protect their eyes from flying particles hazards while installing roofing material with pneumatic (air) powered nail guns while performing roofing work on a residential home. Five Star Roofing CSC LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.102(A)(1) Eye/Face Protection), which was contained in OSHA inspection number 1844224, citation number 1, item number 1 and was affirmed as a final order on 1/2/2026, with respect to a workplace located at 499 Tallwood DR, Norton Ohio 44203.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about January 21, 2026 during the installation of roofing material at a job site located at 5095 Brainard RD, Solon Ohio 44139, the employer did not ensure employees working on a residential roof were protected from a fall of approximately 10.5 feet while working on a residential roof. The employees were working on all sides and edges while removing/installing roofing martial. The owner/foreman was onsite and actively working alongside the exposed employees. Five Star Roofing CSC LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(B)(13) Fall Protection), which was contained in OSHA inspection number 1844224, citation number 1, item number 2 and was affirmed as a final order on 1/2/2026, with respect to a workplace located at 499 Tallwood DR, Norton Ohio 44203.
